Senior Living Source has adapted nursing home rating data from Medicare.gov in an effort to make it more easily searchable. To view selected nursing home ratings in your state, simply enter a zip code or state in the search box below, or select a state from the list of links beneath the search box.

Some things to keep in mind when using these ratings:

• 1 star = far below average; 2 stars = below average; 3 stars = average; 4 stars = above average; 5 stars = no complaints at all.


• Facilities are only surveyed for four to seven days, so ratings may not reflect the quality of care year-round or after improvements are made.


• Complaint ratings may not reflect the quality of smaller facilites, as the rate of complaints per resident will appear higher than the same rate of complaints at a larger facility.
